pbos 081af25c11 Remove kProtectionKey* and VCMKeyRequestMode. 
Enforces previous kProtectionKeyOnLoss as the permanent method which was
the only one used in use. This simplifies SetVideoProtection and
transition over to SetReceiverRobustnessMode.
