NetEqDecoder is still used in the external interfaces, but this change
opens up the ability to use SdpAudioFormats directly, once appropriate
interfaces have been added.

This is still a tiny lie, since it checks for kCodecArbitrary to avoid
scaling a codec with an external decoder, because of missing information
in that case. The main point is still true, though. Once the next CL is
in, removing NetEqDecoder usage completely in DecoderDatabase, another
workaround will be in place for external decoders until we can fully
deprecate them.

The bug hasn't caused us any problems, since we don't run CNG together with Opus (our only real 48 kHz codec), but would cause problems if used with PCB16b @ 48 kHz.

This operation was relatively simple, since no one was doing anything
fishy with this enum. A large number of lines had to be changed
because the enum values now live in their own namespace, but this is
arguably worth it since it is now much clearer what sort of constant
they are.
