Finally we are able to remove this class entirely, along with the last
vestiges of it's use. I've also removed some legacy files that were only
used for windows XP support.

This also deletes unused features of the video_capturer interface, the classes
VideoCaptureFeedBack, VideoCaptureEncodeInterface and related methods,
and the module id which used to be passed as an argument to the
VideoCaptureDataCallback.
In theory the module id could have been used to let a single
VideoCaptureDataCallback serve several capturers, and demultiplex
on the id, but in practice, it was unused. With this change, it is
required to use a separate VideoSinkInterface for each capturer.

It is just a pass through to webrtc/video_frame.h. Updated the callers
to include webrtc/video_frame.h instead and removed i420_video_frame.h.
This should fix pbos' TODO in i420_video_frame.h.

Since frames delivered using VideoSendStream no longer use the external capture module, VideoSendStream will not get an incoming framerate callback. VideoSendStream now uses a rtc::RateTracker.
Also, the video engine must ensure that time stamps are always increasing.
With this, time stamps (ntp, render_time and rtp timestamps ) are checked and set in ViECapturer::OnIncomingCapturedFrame

This fixes a two year old TODO of deleting dead code :)
In cases where the _id or id_ member variable is being used for tracing,
I changed the member to at least be const.
It doesn't look like id's are that useful anymore so maybe the next step is to get rid of them.

This fixes a variety of MSVC warnings about value truncations when implicitly
storing the 64-bit values we get back from e.g. TimeTicks in 32-bit objects, and
removes the need for a number of explicit casts.
This also moves a number of constants so they're declared right where they're used, which is easier to read and maintain, and makes some of them of integral type rather than using the "enum hack".
