Per comments from HL/kwiberg on https://webrtc-codereview.appspot.com/42569004 , when there is existing usage of mixed types (int16_t, int, etc.), we'd prefer to standardize on larger types like int and phase out use of int16_t.
Specifically, "Using int16 just because we're sure all reasonable values will fit in 16 bits isn't usually meaningful in C."
This converts some existing uses of int16_t (and, in a few cases, other types such as uint16_t) to int (or, in a few places, int32_t). Other locations will be converted to size_t in a separate change.

A better solution than forcing OPUS_APPLICATION_VOIP when enabling DTX has been found, which is to set OPUS_SIGNAL_VOICE.
This reduces the uncertainty of entering DTX over silence period of audio.
This CL contains the setup of OPUS_SIGNAL_VOICE and decoupling opus application mode with DTX.

CodecOwner is introduced here; AudioEncoderMutable was introduced in a
previous commit, but had no users until now. The only remaining task
for ACMGenericCodec was to construct and maintain the stack of speech,
CNG, and RED encoders. This task is now handled by the CodecOwner,
which is owned and used by the CodecManager.

This change essentially divides AudioCodingModuleImpl into two parts:
one is the code related to managing codecs, now moved into CodecManager,
and the other is what remains in AudioCodingModuleImpl.
This change also removes AudioCodingModuleImpl::InitializeSender. The
function was essentially no-op, since it was always called immediately
after construction.

I'm splitting the timer functions in EventWrapper into a separate interface.
- Users of the timer functions have different needs than users of a generic event
- Providing a default implementation for EventWrapper that simply uses rtc::Event.
This means that clients of WebRTC that don't use the relatively few classes, typically rendering classes, that depend on the event timer functionality, also don't pull in dependencies on multimedia timers.

In SDP, RED audio codec has its own sample rate. Currently, we offer RED/8000 (8 kHz). But the actual send codec can violate this sample rate. The way to solve it is to introduce more RED payload types, e.g., RED/16000, RED/32000.
As a first step towards that, we, in this CL, limit the current RED (RED/8000) to work only with 8 kHz codecs.

Removes ThreadPosix::InitParams and a corresponding wait for an event.
This unblocks ThreadPosix::Start which had to wait for thread scheduling
for an event to trigger on the spawned thread, giving faster Start()
calls.

Remaining uses of OVERRIDE was fixed by search+replace.
Manual edits were done to fix virtual destructors that were
overriding inherited ones.
Finally a build error related to the pure virtual definitions of
Read, Write and Rewind in common_types.h required a bit of
refactoring in:
* webrtc/common_types.cc
* webrtc/common_types.h
* webrtc/system_wrappers/interface/file_wrapper.h
* webrtc/system_wrappers/source/file_impl.cc
This roll should make it possible for us to finally re-enable deadlock
detection for TSan on the buildbots.

This solution does not use the existing VAD/DTX logic of ACM, since Opus DTX is codec feature, while ACM VAD/DTX is mainly for setting the WebRTC VAD/DTX.
During the development of this CL, two old bugs were found and are fixed in this CL too.
They are in
webrtc/modules/audio_coding/test/Channels.cc
and webrtc/modules/audio_coding/main/acm2/acm_opus_unittest.cc
respectively.

They have all been replaced by AudioEncoder subclasses, accessed throgh
ACMGenericCodecWrapper objects. After this change, the only subclass of
ACMGenericCodec is ACMGenericCodecWrapper. (The two will be consolidated
in a future cl.)
This CL also deletes acm_opus_unittest.cc. This test file was already
replaced audio_encoder_opus_unittest.cc in r8244.

It was not clear how one could know if ACM is using DTX from WebRTC or codec internal DTX.
This CL makes better use of IsInternalDTXReplacedWithWebRtc() which was designed for G.729 to export such information.
Before
IsInternalDTXReplacedWithWebRtc() gives true only if codec == G729 and G729's internal DTX is replaced with WebRTC DTX.
Now
IsInternalDTXReplacedWithWebRtc() gives true also when codec does not have internal DTX, i.e., must use WebRTC DTX, which is much more logical.

r7049 added some unnecessary casts ("return 0" -> "return static_cast<uint16_t>(0)"). r7123 converted these to "return 0u". The original impetus for this was to eliminate type conversion warnings. However, the 'u's are unnecessary; Visual Studio can return "0" from a function returning an unsigned value without producing a warning. The real reason for the original warnings was that the code was returning -1 from a function returning an unsigned value, which does need a cast; the -1s were eliminated before the above two revisions landed.
Also reverse the order of some conditionals to prevent possible underflow.
While the underflow wouldn't have changed the behavior of the code, it's easier
to reason about the code when such underflow can't happen, and possibly safer
against future modifications as well.

RTPStream, and NetEq as such. Also mark all other virtual overrides in the same
files.
This will make further changes to these classes safer by ensuring that the
compile breaks if the base class changes and not all overrides are fixed.
This also deletes ACMTest.cc, which existed solely to define ~ACMTest(), which
was marked pure virtual in the header. (Pure virtual destructors still need a
definition.) Because there is another pure virtual method in this class, the
class is already abstract, so there's no benefit to making the desturctor pure.
Making it non-pure allows removing the separate source file.
