These legacy endpoints were supported with Plan B since the SDP
parser would fill in default MID values if a=mid was absent that
happened to match the default offered MIDs.
With Unified Plan, these default MIDs changed so the autofilled
MIDs do not match any more.
This CL adds information to the SessionDescription struct to
indicate whether or not a=mid was present and modified
PeerConnection::SetRemoteDescription to copy MIDs from the local
description if the a=mid lines are not present.

This changes all internal code to use the media_description() helper
for ContentInfo along with the as_audio, as_video, and as_data casting
methods introduced in a previous CL. Reduces the total number of
pointer static_casts in pc/ from 351 to 122.

SDP is a detail of PeerConnection and is not used by anything in p2p, so
it belongs in the pc/ directory. This also allows
MediaContentDescription to be inlined in the future.
